STEPS TO STARTING THE FOSTER CARE/ ADOPTION LICENSING PROCESS
STEP 1: VIRTUAL INTRODUCTORY AGENCY ORIENTATION on Zoom *REQUIRED TO MOVE FORWARD

STEP 2: FINGERPRINTING
All adults in the home (18+) must apply for an AZ DPS Level One Fingerprint Card. If you already have a DPS Level One Clearance Card please skip this step. It is important to follow the PDF instructions “Step-by-Step.” Please email us at: [email protected] for Fingerprint Instructions.
STEP 3: ACCESS TO STATE OF ARIZONA QUICK CONNECT LICENSING PORAL
This link will redirect you to encrypted Jotform application to Quick Connect Licensing Portal to apply for the Family Foster Home License: https://www.jotform.com/assign/212384987882069/243450900128045
STEP 4: AGENCY INITIAL HOME VISIT
Complete the Agency initial home visit in your home (plan for one hour).
- We will come to your home by appointment only.
- We will go over all foster care licensing documents.
- We will begin the licensure interview process.
- We will complete mock Life Safety Inspection (LSI) of your home. We will inform you of all corrections needed to pass the State LSI.
- We will answer all your questions.
The licensing Agency will provide you for registration steps at Step 4. All training is virtual except for CPR Training.
